About the role

As a Support Coordinator - Level 2 at Your Choice Support Coordination, you will play a crucial role in assisting clients with complex needs to access and coordinate the support services they require. Based in Kirwan, Queensland, this full-time position is responsible for ensuring clients have the necessary support to live independently and achieve their personal goals.

What you'll be doing

Undertake comprehensive assessments to identify client's needs, goals and preferences

Coordinate and link clients to a range of support services, including allied health professionals, accommodation and community participation

Assist clients to develop, monitor and review their NDIS plan

Advocate on behalf of clients to ensure they receive appropriate funding and support

Maintain detailed case notes and provide regular progress updates

Build and maintain strong working relationships with service providers

Participate in team meetings and professional development opportunities

What we're looking for

Relevant qualification in disability or community services

Minimum 2 years' experience in a similar support coordination or case management role

Proven ability to work collaboratively with clients, their families and a range of service providers

Excellent communication, interpersonal and problem-solving skills

Understanding of the NDIS and ability to navigate the system on behalf of clients

Commitment to person-centred, strengths-based practice

Current driver's licence and reliable vehicle
