About the role
Permanent and temporary opportunities to be responsible for the provision of a broad range of clinical pharmacy services to the facilities within the community and oral health (COH) community services or facilities on a rotational basis and that align with organisational goals.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Deliver evidence based clinical pharmacy services appropriate to the model of care associated with each particular COH community and bedded services. This includes, but not limited to, community palliative care services, transition care programs, post-acute care services and residential transition programs.
- Work collaboratively and effectively within not only the pharmacy team, but wider multidisciplinary team and with patients / carers.
- Provide clinical education pertaining to routine tasks for students and clinical practice supervision to less experienced staff with the support of senior clinical staff.
