The opportunity
Vacenti is seeking a Full-time Clinical System Coordinator (Maternity Leave Cover- 12 month) to join the Clinical Team at Shared Services. Reporting to, and working closely with the Clinical Systems Manager, In this role you will be responsible for assisting Clinical Systems Manager to support facilities in achieving maximum aged care funding revenue and quality care outcomes from effective clinical system management.
The Clinical System Coordinator works collaboratively with facility clinical team to ensure required documents such as clinical assessments and documentation are prepared for care funding claims within the required timeframes and will assist care workforce knowledge and skills in documentation content and requirements of care funding management. The Clinical System Coordinator is directed by Clinical System Manager and supports management and maintenance of clinical systems and procedures with effective strategies.
Your main responsibilities will include:
Ensure all homes are completing required clinical assessment and documentation as required for funding claims.
Monitor residents for changes to care needs and discuss with FM/CM and Clinical Systems Manager for possible care funding reclassification.
Liaise fortnightly (as a minimum) with Clinical Systems Manager and key managers to discuss care funding activity, resident care need changes, potential reassessment, and other relevant information.
Ensure that all clinical documentation reflects resident’s current care needs and personalised strategies in a holistic care approach.
Assist each site to ensure staff are educated in the provision of accurate and quality assessment and reliable data relating to resident care and funding management.
To be successful in this position you will need:
Minimum 2 years’ experience in Aged Care as Registered nurse.
Experience in ANACC funding systems and processes along with working knowledge of Aged Care Quality Standards.
Excellent clinical assessment and care planning skills.
Excellent problem solving, time management and organisational skills.
High level of competency with office management systems and MS office applications, including Word, Excel, and Outlook.
Ability to coach and mentor staff and assist with staff training.
Ability to think proactively, innovatively, and laterally to maximise opportunities for care funding and improvement and promote the work of the Home.
Ability to effectively communicate and work in collaboration with care staff team.
