About the role

We are seeking a clinically experienced and motivated professional to provide leadership and oversight of complex health supports across our Supported Independent Living (SIL) services.

Based in Darwin, the Clinical Care Coordinator plays a key role in ensuring participants with complex health needs receive safe, person-centred, and high-quality support. The role includes regular travel to Somerville’s SIL services in Palmerston, Katherine, and Alice Springs, supporting teams and strengthening clinical practices across regions.

This hands-on, Monday to Friday role offers professional variety, autonomy, and the opportunity to use your clinical expertise to build team capability and improve outcomes for people with disability.

What you’ll be doing

As Clinical Care Coordinator, you’ll lead clinical risk management, care planning, and practice development across multiple SIL houses. You’ll also coach frontline staff, promote safe and person-centred practices, and collaborate with health professionals and families to ensure integrated, responsive care.

Key responsibilities include:

  • Conducting clinical assessments and developing person-centred care plans
  • Overseeing high intensity supports and managing complex care needs
  • Coaching and supervising support staff to ensure clinical safety and confidence
  • Facilitating referrals and building strong links with health professionals
  • Monitoring clinical risk, incident response, and documentation compliance
  • Contributing to service improvement, audits, and policy development
  • Working closely with families and guardians to support quality outcomes

What you’ll bring

You’ll bring clinical expertise, innovation, and critical thinking, to engage and support staff to deliver safe, high-quality care in a disability support environment.

You’ll also need to demonstrate:

  • Registration as a Registered Nurse or Allied Health Professional (AHPRA or relevant body)
  • Knowledge of high intensity supports (e.g. enteral feeding, catheter care, seizure management)
  • Strong clinical judgement and communication skills
  • Ability to supervise, coach, and support the development of frontline staff
  • Commitment to person-centred care and human rights
  • Excellent documentation, organisation, and time management skills
  • Understanding of NDIS Practice Standards and safeguarding requirements

You will also need to hold (or have the ability to obtain):

  • A current Ochre Card
  • National Criminal History Check
  • NDIS Worker Screening Clearance
  • First Aid and CPR certificate
  • COVID-19 and Influenza vaccinations
  • Australian work rights
  • NT Driver Licence
