Are you passionate about making a meaningful difference in the lives of veterans and their families? We are seeking an experienced Clinical Care Coordinator to join a federal government agency dedicated to supporting the mental health and wellbeing of current and former Australian Defence Force personnel.
In this role, you will provide evidence-informed, short-term clinical case management to clients facing complex mental health and psychosocial challenges. You will work collaboratively with internal teams and community partners to remove barriers to care and promote engagement in mental health services.
Full-time role, 37.5 hours per week
24 months with possible extension options

SOME KEY RESPONSIBILITIES
A person in this role will deliver high-quality clinical case management services to clients with complex psychosocial needs. This includes risk assessment, brief intervention, care coordination, and referral to ensure clients engage effectively with mental health care and support services. Working collaboratively with government, ex-service organisations, and community agencies, you will contribute to improving the health and wellbeing of vulnerable veterans and their families.
Provide high-quality, evidence-informed clinical services including risk management, brief interventions, care coordination, and referrals for clients with complex and multiple needs.
Maintain accurate and confidential client records using the agency’s client management system, including session notes, care plans, and risk assessments.
Develop and maintain partnerships with government departments, ex-service organisations, community mental health, and welfare agencies to support client care.
Collaborate with internal and external stakeholders to promote recovery-oriented, stepped care models for vulnerable veteran and defence force communities.
Participate in continuing professional development and clinical supervision to maintain professional registration and clinical standards.
Adhere to relevant legislation, ethical guidelines, and organisational policies to ensure best-practice client care.
Attend regular case review meetings with clinical teams and management to discuss client progress and care extensions.
Facilitate group treatment and psychoeducation programs alongside other clinical staff or external clinicians.
Contribute to community development, service promotion activities, and program evaluation to improve clinical service delivery.
Support quality assurance activities to ensure clinical practice and reporting meet contractual and organisational requirements.
Assist in managing client intake and allocation processes through liaison with clinicians and administrative staff.
REQUIREMENTS
Relevant qualification in Psychology, Occupational Therapy, Nursing, Social Work, or Counselling at Masters level
Current professional registration with AHPRA, AASW, PACFA, or ACA at required levels
Demonstrated experience in clinical case management and client-centered care
Strong interpersonal and communication skills with the ability to build effective partnerships
Commitment to continuous professional development and adherence to ethical guidelines
Ability to work both independently and collaboratively within a multidisciplinary team
Eligibility to obtain a Baseline security clearance and meet pre-engagement screening requirements
