About the role

As a Support Coordinator Level 3 and Psychological Coach at All About Recovery Pty Ltd, you will play a vital role in enabling participants to achieve their goals and live their best life. Based in Penrith, NSW, this full-time position will see you coordinate a range of funded supports and services to assist participants with complex needs.

What you'll be doing

Develop, coordinate and monitor complex support plans for participants with high-level needs

Liaise with a range of service providers, including Allied Health professionals, to ensure seamless service delivery

Advocate on behalf of participants to access relevant, quality services and supports

Regularly review and update participant plans to ensure they remain relevant and effective

Maintain detailed, accurate records and reporting on participant progress

Participate in team meetings and contribute to the continuous improvement of service delivery

What we're looking for

Relevant tertiary qualification in a human services field (e.g. Social Work, Disability, Allied Health)

Minimum 3 years' experience in a similar support coordination or case management role

Excellent communication and interpersonal skills, with the ability to build strong relationships with a diverse range of stakeholders

Deep understanding of the NDIS and experience working within the scheme

Demonstrated capacity to work autonomously and as part of a team

Proficient in using client management systems and data reporting

Current driver's licence and access to a reliable vehicle
