We are seeking dynamic, experienced, and motivated individuals to join our team as a Support Professional Team Leader in Newmarket.

The successful candidate will play a pivotal role in overseeing and leading a team of support professionals dedicated to providing high-quality care to our clients. This is an exciting opportunity to contribute to a rapidly growing organisation and to support the development and well-being of individuals in the community

Role Description

• Assist people to live as independently as possible in their own home and in the community, remove barriers and engage in a full life;
• Personal care including continence care, mealtime management, helping people shower, getting dressed and performing daily activities, laundry etc.
• Assisting with light housework;
• Providing social support to people with disabilities and people experiencing mental health;
• Assist people with physical and psycho-social disabilities to remain independent in their own homes;
• Supporting clients to appointments in the community.
• Providing therapy assistance to support the implementation of physical, speech, OT and psychological plans and strategies (as directed by their allied health teams)
• Supporting people to develop and build self-confidence;
• Lead, mentor, and support a team of support professionals to ensure the delivery of high-quality care services to clients.
• Foster a positive, inclusive, and respectful work environment that encourages collaboration and professional development.
• Oversee the day-to-day operations of support services, ensuring they align with NCHHS’s values and best practice standards.
• Work closely with clients and their families to assess needs and develop individualized care plans.
• Monitor and review client progress, ensuring the delivery of services is in line with agreed outcomes.
• Provide guidance and support to team members on complex cases and situations.
• Maintain accurate documentation and reporting in line with NDIS and other regulatory standards.
• Participate in training and development programs to stay current with industry best practices.

Qualifications and Experience

We are looking for individuals with a passion for supporting others and strong leadership skills.

The ideal candidates will possess:

• Proven experience in a leadership or supervisory role within the health and disability sector.
• A strong understanding of the NDIS and its operational requirements.
• Excellent communication, interpersonal, and conflict resolution skills.
• A commitment to continuous professional development.
• Ability to work autonomously as well as part of a collaborative team.
• A strong focus on person-centered care and holistic health principles.

Essential

• Current QLD Driver’s License
• Current First Aid and CPR Certificate
• Current Yellow Card/NDIS Workers Screening Check
• Compulsory Comprehensive Insurance on private vehicle
• NDIS Orientation Certificate or ability to obtain.

Desirable

• Certificate III in Individual Support Disability/Aged Care or related studies (or willing to undertake)
• Knowledge and understanding of the NDIS system.
• Sound experience in the disability or allied health/welfare/education fields.
• A minimum of one year's driving experience and a full driving license.
• Currently located in Brisbane, Ipswich, Logan, Bayside, Gold Coast and the Sunshine Coast.
