THE ROLE
We have a permanent, full-time opportunity for a Community Coordinator (RN) to join our fantastic and friendly team. Our Community Coordinators play a vital role in the daily lives of our clients, creating a supportive environment with a focus on wellbeing.
The successful candidate will be required to support our prospective and current senior clients in navigating the full cycle of their care package. From initial meeting through to navigating the My Aged Care system, you will work with your clients to develop goals that promote choice, control, and independence. These goals drive the development of individualised budgets and support plans, allowing clients to thrive in their own homes.
You will conduct regular in-person reviews with clients, allowing you to build high levels of rapport and trust that help you to understand and assess current and future needs, ensuring maximum support and prompt identification of necessary changes to care requirements.
Based at our centrally located Park Road office in Milton (4064), this team-oriented role offers a dynamic mix of collaboration and independence. You'll enjoy working autonomously while staying mobile, visiting clients and providing valuable support to our dedicated carers in the community. The coverage area may include suburbs in the Brisbane central and Brisbane northside (to Redcliffe) areas, with a company vehicle provided. (Occasionally, there may be a requirement to travel to other locations based on business needs, so flexibility is also important).
This role will be best suited to a committed and dedicated Registered Nurse who embraces autonomy and enjoys working as part of a collaborative team and resides in or around Brisbane North.
We’re looking for an individual who finds genuine joy in supporting and engaging with older adults, someone who is passionate about making a meaningful impact in the lives of seniors and truly values the chance to support others in living their best lives.
Training will be provided to the successful candidate on the ‘My Aged Care’ process (including Home Care Packages/Support at Home).
ABOUT YOU:
To be successful in this position you will be someone who has:
A genuine, warm and friendly disposition, with a passion for enhancing the quality of life, dignity and independence of Seniors/Aged clients in the community.
Excellent interpersonal and communication skills, including time management and attention to detail.
An ability to thrive autonomously and also as part of a collaborative and supportive team.
Current AHPRA registration - Registered Nurses only.
Current First Aid and CPR certificate/s.
Australian or New Zealand citizenship or permanent residence.
A current and valid, OPEN Australian Drivers Licence.
A willingness and flexibility to travel to client locations day-to-day, using a company vehicle.
