About the role

Angel Disability Supports Pty Ltd is seeking an experienced NDIS Support and Services Coordinator to join our team in Telopea, NSW. In this full-time role, you will be responsible for coordinating and delivering high-quality support services to our clients with disabilities, helping them achieve their personal goals and live more independent lives.

What you'll be doing

Develop and implement individualised support plans for clients, in collaboration with them and their families

Coordinate the delivery of a range of support services, including personal care, social and community engagement, and skills development

Monitor and review client progress, adjusting support plans as necessary to ensure optimal outcomes

Liaise with the NDIA, healthcare professionals, and other stakeholders to ensure seamless service delivery

Manage and support a team of support workers, providing guidance, training, and performance feedback

Maintain accurate client records and prepare reports as required

Contribute to the continuous improvement of our services and promote a positive, client-centred culture

What we're looking for

Coordinate supports and services in line with NDIS participant goals

Build and maintain strong relationships with participants, families, providers, and other stakeholders

Use NDIS PRODA and PACE systems to manage and monitor participant plans

Submit comprehensive case reviews and reports for NDIS participants

Liaise with the NDIA, support workers, therapists, and community organisations

Keep accurate records and maintain compliance with NDIS requirements

Manage participant plans using ShiftCare for scheduling and reporting

Relevant tertiary qualification in Disability Services, Social Work, or a related field

Minimum 2 years' experience in a similar NDIS support coordination or case management role

Excellent communication and interpersonal skills, with the ability to build trust and rapport with clients and their families

Strong problem-solving and decision-making skills, with a focus on achieving positive outcomes for clients

Proficient in using client management systems and data reporting tools

Thorough understanding of the NDIS framework and the ability to navigate related policies and procedures

Compassionate and empathetic approach, with a genuine commitment to supporting people with disabilities

Current First Aid certification and a valid driver's licence
