MCWA is a in-home aged care provider with 40 years experience that focuses on supporting the multicultural ageing and elderly communities in Australia. We are currently seeking compassionate and dedicated Case Manager who can speak Italian language to join our team based in the Melbourne, Australia.

Exciting opportunity for a person who is passionate about aged care, and supporting the ageing and elderly. The successful candidate will have a strong client service focus with demonstrated ability to maintain strong ethics and care.

Some of the responsibilities for this position include:
Demonstrated ability to build strong partnership relationships with client
Provide comprehensive case management for each client
Build strong Care Plans that are appropriate for the individual needs of each client whilst meeting the limitations of each client’s budget
Re-assess client care plans, goals, OHS assessment, and emergency plans for clients
Maintain strong client notes
Strong problem solving skills
Nimble in their ability to support clients
Recognise, anticipate, and focus on specific client needs and advocate where required
Work with the Finance team to accurately submit client claims to Services Australia
Approve monthly client statements prior to mail out
Establish, coordinate and manage third party services for clients
About You
Ability to speak Italian language is required
Experience in a similar role
Registered Nurse looking for a change
Proficiency in computer skills
Excellent communication and interpersonal skills
Ability to work independently and as a part of a multidisciplinary team
Valid driver's license
