Join Our Dynamic Team:

We’re expanding our Home Care Packages (HCP) team and looking for a compassionate, people-focused Care Manager (with HCP experience) or Care Coordinator (no prior HCP experience required) to join us in Greensborough. Whether you're an experienced aged care professional or looking to grow into a managerial role, we’ll support your development every step of the way.

Key Responsibilities:

Conduct assessments of new clients to understand their individual goals, needs, and preferences.

Facilitate the onboarding process for new clients, ensuring a smooth transition into the care program.

Develop and implement person-centric care plans in collaboration with clients.

Monitor the ongoing effectiveness of care plans, adjusting as needed to address changing client needs.

Conduct risk and vulnerability assessments of clients to identify potential safety concerns.

Implement strategies for ongoing monitoring of clients' safety and well-being.

Coordinate and schedule client and care team member schedules, ensuring optimal service delivery.

Collaborate with the team to manage and allocate resources efficiently.

Demonstrate a positive attitude towards older individuals, understanding their needs, and upholding their rights and cultural diversity.

Acting as the first point of call for clients, families, other nominated representatives, healthcare professionals and other external providers.

Communicate effectively with clients, families, and the care team, fostering a supportive and respectful environment.

Participate in regular care reviews with clients, families, and other healthcare professionals.

Ensure compliance with relevant regulations and standards.

Stay informed about updates to industry standards, including the Aged Care Act and Aged Care Quality Standards.

Foster a culturally responsive and inclusive environment within the team.

Collaborate with community partners and organisations to enhance service delivery

Working closely with care and support staff members to ensure services are safe, individualised and effective

Work with the broader team on quality and continuous improvement initiatives

Ensure all documentation for clients are accurately maintained and draft reports

Qualifications and Skills:

Experience in community care delivery preferable.

HCP program knowledge.

Strong understanding of consumer-directed care principles.

Cultural competence and experience working with diverse communities.

Excellent time management and organisational skills.

Ability to work autonomously whilst being a team player.

Knowledge of relevant regulations and standards in the aged care will be considered favourably

Passion for socially responsible and person-centric care.

High level of interpersonal skills and ability to build strong and trustworthy relationships.

Demonstrated competence in general computer applications and ability learn our care management software package.

An Australian Driver’s Licence.

Location:

This role is based in Greensborough, VIC and requires the successful candidate to work from our Greensborough office. The role also requires occasional travel around Melbourne Metropolitan Area to visit our clients and prospective clients.
