About the Role

We are seeking a passionate and experienced Clinical Home Care Team Leader to join our Home Care team (Northwest). This leadership role is responsible for overseeing clinical and care management services for Home Care Package and CHSP participants. You will lead a multidisciplinary team and ensure high-quality, person-centred care aligned with best practice and regulatory standards.

Key Responsibilities

Lead and supervise Care Partners, Clinical Care Partners (RN) and CHSP Care Coordinators (Northwest team)

Conduct assessments, care planning, and clinical reviews

Ensure compliance with Aged Care Quality Standards and clinical governance

Manage referrals and collaborate with internal and external stakeholders

Contribute to audits, risk management, and continuous improvement initiatives

Support professional development and team performance

Key Requirements

Essential Qualifications

Bachelor’s degree in Nursing

Current AHPRA registration

Experience & Skills:

Minimum 3+ years nursing experience in aged or community care

Strong leadership and team management skills

Experience in care planning, budget monitoring, and clinical oversight

Knowledge of aged care reforms, Person Centred Care, and relevant legislation

Excellent communication, problem-solving, and organisational skills

Community second language – highly regarded
