Position Overview: As a Case Manager at TDN, you will play a pivotal role in coordinating services for a diverse range of clients. Your primary objective will be to support recipients of Home Care Package funding who aspire to maintain their independence and live comfortably in their own homes.

What We're Looking For: We are seeking dedicated, client-focused individuals who possesses the necessary experience, knowledge, and skills. You should be energetic, have the ability to collaborate with various healthcare providers and team members.

Key Responsibilities:

• Conduct integrated assessments to understand the unique needs of each client.

• Collaborate with a variety of healthcare professionals and service providers to coordinate an array of services.

• Establish and maintain strong relationships with key stakeholders involved in the client's services.

• Ensure that government funds are efficiently utilised within the bounds of legislation and guidelines.

Key Requirements:

• Exceptional organisational skills

• Commitment to working within a multidisciplinary team

• Proficiency in verbal and written communication

• Strong computer skills, particularly with Microsoft Suite and client management systems

• Able to self-manage and work independently
