About the role

As a full-time or part-time Clinical Case Worker / Social Worker in Mental Health, you will be joining our multidisciplinary team at Rural Health Tasmania Inc. in Devonport, Tasmania. In this role, you will provide essential mental health and social support services to our clients in our office and community setting, working collaboratively with our team of healthcare professionals to deliver holistic, person-centred care to clients with complex needs.

What you'll be doing

Consult and liaise with the team and Team Leader in the development and implementation of health promotion activities in collaboration with local services and community, this includes internal organisational programs, considering group programs run by SCMHS team;

Implementing individualised care plans to address the unique challenges faced by clients

Engage and liaise with local GP practices, psychiatrists, psychologists, psychosocial and allied health services as well as internal services using the stepped model of care provided;

Maintaining accurate and up-to-date client records and contributing to the team network

What we're looking for

Relevant experience in community services;

Social work Degree or equivalent;

2-5 years’ experience in community settings;

Care coordination and/or case management experienceDemonstrated experience in delivering mental health and/or social support services, preferably in a community-based or primary care setting;

Strong clinical assessment and case management skills, with the ability to work collaboratively within a multidisciplinary team;

Familiarity with relevant legislation, regulations, and best practices in the healthcare and social services sectors.
