Location: Cloverdale, Perth WA

Position: Full time

Package (Annual): $70,000 - $75,000 + $11,000 on-call (pro-rata basis) + 11.5% Super

The Star Care Group, one of Western Australia's leading providers of Community Support and in-home disability support, is actively seeking an experienced Services Coordinator/ Team Leader to join our diverse team as a result of business growth. This position plays a vital role in assisting The Star Care Group in delivering essential services to our participants while ensuring efficient service provision. As a member of our team, the Services Coordinator/Team Leader establishes strong relationships with both The Star Care Group's staff and clients, facilitating effective communication through phone and face-to-face interactions to address roster-related matters. We are looking for a candidate who is bright and full of energy, solution-oriented, data-driven, possesses a keen eye for detail, and excels in communication. Demonstrated leadership capabilities are essential, with the ability to motivate, mentor, and guide a high-performing rostering team. Establish and maintain effective relationships with clients and their families, providing ongoing support and guidance.

Key responsibilities of this role include:

Act as a trusted point of contact for clients and their families, addressing any concerns, questions, or feedback.

Be a part of developing and overseeing, implementing and reviewing of individualised client care plans.

Ensure that all services are delivered as per NDIS guidelines and meet the specific needs of each client.

Manage and lead the rostering team, ensuring that client needs are matched with appropriate support staff.

Develop and implement effective rostering practices to maximize staff utilisation and client satisfaction.

Monitor and document client progress and outcomes, ensuring all necessary reports are completed and submitted in a timely manner.

Provide regular updates to management on client status, service delivery quality, and any areas requiring improvement.

Ensure all services meet NDIS standards and comply with relevant policies and procedures.

Conduct regular audits and reviews to maintain high standards of care and service delivery.

Oversee the incident reporting process, ensuring timely and accurate reporting and documentation of any incidents.

Investigate and fill the incident register when necessary.

Implement follow-up actions and improvements to prevent future occurrences.

Prepare and complete progress reports for each client, outlining their achievements, challenges, and recommendations for further support.

Work collaboratively with other team members to ensure cohesive and comprehensive support for clients.

Foster a positive team environment, encouraging open communication, continuous learning, and mutual support.

Adhere to company policies and procedures.

Any additional duties to assist the rostering team including roster drafting, updating, giving instructions to the team, training new members etc.

Assist or be a part of conflict resolution within the team.

Organise fortnightly meetings with internal team to discuss and solve challenges.

What we're seeking with our successful candidate:

Experience coordinating care in disability or community services, including developing and implementing individualised care plans.

Strong understanding of the NDIS framework, particularly in support coordination and client-focused care delivery.

Incident reporting and risk management expertise, including assessment and mitigation strategies.

Proven leadership skills, with previous experience managing rosters and leading teams in a fast-paced, client-centric environment.

Knowledge of compliance standards, quality improvement processes, and audit readiness.

Excellent documentation and reporting skills, with the ability to track client progress and prepare detailed reports.

Outstanding communication and interpersonal abilities, with a natural talent for building rapport with clients and families.

Top-notch organisational and time management skills, able to handle multiple priorities and deadlines with ease.

Confidence using Microsoft Office and other relevant scheduling/reporting software.

Critical thinking and problem-solving ability, especially when managing incidents and adjusting care plans on the go.

Essential Selection Criteria:

Qualifications in Social Work, Disability Services, Community Services, or a related field (preferred).

Current NDIS Worker Screening Check.

Australian Citizenship or full working rights to live and work in Australia.

Current National Police Check (or ability to obtain one prior to commencement).

COVID-19 vaccination.
