Remote Social Worker / Case Manager – Veteran Health

We are seeking qualified Social Workers who are passionate about supporting the health and wellbeing of Australian veterans. This fully remote role provides the opportunity to deliver meaningful psychosocial support, case management, education, and advocacy, all within a structured and supportive national health program.

The Opportunity
As a Remote Social Worker / Case Manager, you will play a key role in providing structured case management, psychosocial support, and therapeutic interventions to a caseload of veteran clients. You will support clients through wellness check-ins, education, advocacy, and assistance navigating the Department of Veterans' Affairs (DVA) and other eligible services, all with a focus on improving overall wellbeing.

This position requires full-time availability, Monday to Friday, between 8:00am – 5:00pm, with the flexibility to work entirely from home.

Key Responsibilities

• Deliver high-quality case management and psychosocial support
• Conduct wellness check-ins, provide education, and advocate for client needs
• Assist clients in navigating DVA processes and accessing eligible services
• Provide therapeutic interventions aligned with best practice standards
• Maintain compliance with AASW professional guidelines and ethical service delivery
• Work independently while meeting reporting and quality assurance requirements

About You

• Bachelor’s degree or higher in Social Work
• Accredited member of the AASW (General or Mental Health Accreditation)
• Experience in private practice or delivering remote psychosocial services
• Independent contractor with an ABN and appropriate insurances
• Australian Citizen or Permanent Resident
• Proven experience in case management, mental health, veteran support, and/or chronic care
• Ability to work autonomously with excellent time management and communication skills
• Dedicated home office setup with reliable internet connection
