We’re seeking a Clinical Case Manager to help deliver truly person-centred care where clients remain at the heart of every decision and where your nursing practice will contribute to clinical care, advocacy, and life-enhancing support.

Hourly Pay Rate:
· RN: $45.85 (1st Year RN2) to $52.11 (6th Year RN2)
About you
You’re an experienced Registered Nurse with strong aged care expertise. You communicate clearly, solve problems with empathy, and embrace person-centred care. You’re ready to collaborate with diverse teams and you understand the value of community connections in supporting older people to live well at home.
Experience in community nursing, case management, or care coordination is a plus but not essential. We value the right mindset, adaptability, and a genuine passion for positive ageing.
Responsibilities as a Clinical Case Manager
Conduct comprehensive client assessments and develop personalised care plans aligned with Home Care Packages and Short-Term Restorative Care funding programs.
Work with healthcare providers, community services, and families to ensure high-quality, seamless care.
Proactively track client health and well-being, identifying changes and facilitating timely interventions and referrals.
Empower clients to make informed decisions about their care, respecting their preferences and promoting independence.
Provide clear guidance to clients and families on health management, medication, and lifestyle choices.
Maintain accurate, compliant records of assessments, care plans, and interventions.
