Country Connect Foundation is a First Nations-led not-for-profit organisation dedicated to delivering culturally grounded disability and wellbeing services in remote Aboriginal communities across the Northern Territory. The Role

We are seeking a passionate and experienced Senior Support Worker to join our team. This role combines direct support work with leadership and coordination responsibilities, focusing on mentoring staff, organising group activities, and collaborating with allied health professionals to deliver holistic, culturally safe support.

Key Responsibilities:

Mentorship & Supervision:

Provide guidance and support to a team of support workers, fostering professional growth and ensuring high-quality service delivery.

Conduct regular team meetings and debriefs

Program & Activity Coordination:

Design and implement regular group activities and programs that promote community engagement, skill development, and cultural connection.

Organise and lead bush trips, cultural outings, and social events.

Collaboration with Allied Health:

Work closely with allied health professionals to develop and facilitate skills-based collaborative activity sessions tailored to client needs.

Ensure integrated support plans that align with therapeutic goals.

Direct Support:

Assist clients with personal care, meal preparation, and daily living activities.

Support clients in residential settings, ensuring a safe and comfortable environment.

Facilitate community engagement and return-to-country initiatives.

Maintain accurate client records and documentation in line with organisational policies and procedures.

Navigate remote areas safely, which may involve 4WD travel.

About You:

Essential Criteria:

Proven experience in a senior support role, with demonstrated leadership and mentoring capabilities.

Strong understanding of culturally safe practices and experience working in remote Aboriginal communities.

Excellent communication and interpersonal skills.

Ability to work independently and as part of a cross-cultural team.

Proficient in coordinating programs and activities that promote community engagement.

Experience collaborating with allied health professionals to deliver integrated support.

Valid NT Driver’s License.

Working with Children Check (Ochre Card).

NDIS Worker Screening Check (or willingness to obtain).

First Aid & CPR Certificate (or willingness to obtain).

Desirable Criteria:

Lived experience in disability, caregiving, or community work.

4WD driving skills and confidence in off-road travel.

Qualifications in disability support, aged care, youth work, or community services.
