About us Grand Pacific Health (GPH) is a for-purpose community-based organisation. GPH provides high quality health services to assist people in the management of their physical and mental health. GPH’s mission is to provide equitable heath care with an emphasis on helping vulnerable and at risk of disadvantage populations as well as regional communities. GPH employs over 400 staff including many qualified health and other professionals including peer workers and community workers. GPH operations cover a large regional geographical footprint in NSW and ACT. About the position The Care Navigator works collaboratively with consumers to connect them with services and supports in the community, facilitating person-centred and integrated care. This position delivers practical support through information sharing and education, identifying and addressing barriers to access and facilitating warm referrals, ensuring consumers are linked to the right service at the right time. This position also works collaboratively with the other members of the team and external service providers to maximise care outcomes for consumers. Based in Wollongong, this is a part-time opportunity (22.5 hours per week), fixed-term to 30 June 2026. We are looking for someone with:

1. A tertiary qualification in a relevant discipline, and/or extensive experience in mental health, health, community services or social sciences.
2. Demonstrated understanding of the health and wellbeing needs of consumers with mental health and/or substance use problems.
3. Demonstrated understanding of supports and services available in the local and regional communities.
4. Demonstrated ability to engage with consumers and their families or carers to provide care navigation and referral support.
5. Current driver's license and willingness to travel as part of role.
