As reforms roll out under the Support at Home program, we're urgently seeking a resourceful, ambitious, and compassionate Care Partner to join our team. This is a full-time opportunity for someone who loves a challenge, thrives on coordinating diverse services, and is ready to drive program growth while supporting Home Care Package (HCP) recipients.

About the Role
You'll coordinate all eligible services under the Support at Home framework—nursing, allied health, home support, equipment, etc.—helping clients navigate their packages and exercise choice and control. Travel is required across the Gold Coast, Brisbane, and Sunshine Coast using a company vehicle.
Your Responsibilities
Manage a caseload of HCP clients, adapting care plans to suit evolving Support at Home guidelines
Coordinate services including allied health, nursing, home support, equipment, and more
Educate and guide clients on package usage, budgeting, and decision-making
Drive program growth via community outreach and referral pathways
Monitor service quality, client outcomes, and budget compliance
Mentor colleagues in best-practice case management and compliance
About You
Proven experience in aged care case management (HCP/CHSP preferred)
Minimum 2 years' experience in aged care case management (HCP/CHSP preferred)
Skilled coordinator, planner, and client advocate—including navigating reform
Nursing qualifications (RN or EN) highly desirable
Strong communicator, emotionally intelligent, results-driven
Able to work independently while part of a broader multidisciplinary team
Passionate about delivering quality care and achieving meaningful outcomes
Must be eligible for National Police Check and have valid Working with Vulnerable People clearance
Valid Australian driver's licence and clean driving record required
