We’re now looking for a passionate and experienced Care Manager to join our Home Services team and help us continue delivering quality, client-focused care to older Australians through our Home Care Packages.

The Opportunity

As the Care Manager Home Services, you’ll play a vital role in ensuring our Home Care Package (HCP) clients receive the support they need to remain independent and safe in their homes. You’ll lead a team of dedicated Support Workers, coordinate care plans, and build strong relationships with clients, carers, and partners — all while upholding aged care standards and our YellowBridge values.

What You’ll Do

Coordinate individualised, high-quality home care services for HCP clients

Develop and monitor care plans, budgets, and reporting

Support, mentor and manage a team of Support Workers

Build and maintain strong relationships with clients, carers, and service partners

Ensure services comply with Aged Care Quality Standards, WHS, and best practice

Represent YellowBridge in sector meetings and forums

Collect and manage relevant client data and contribute to reporting and quality improvement

What You’ll Bring

Minimum Certificate IV in Aged Care or equivalent qualification

Strong understanding of Home Care Packages and aged care sector requirements

Proven experience in case management and leading community-based teams

Excellent interpersonal skills and ability to build trust with clients and families

Strong time management and data collection skills

A positive, client-centred mindset and passion for helping people thrive
