About Us
Apunipima Cape York Health Council is a community controlled Aboriginal and/or Torres Strait Islander health service, which provides culturally appropriate, family centred, primary health care services to the communities and people of Cape York.
About the Opportunity
The Elder Care Support Connector is responsible for connecting with local Elders and older Aboriginal and Torres Strait Islander people and their families to raise awareness of their aged care entitlements to ensure that support for clients is organised, sequenced and supported to enable effective access to appropriate aged care services. The position is full time, fixed term with an end date of 30 June 2026.
There is a genuine occupational requirement under Section 25 of the Anti-Discrimination Act 1991 (Qld), for this position to be filled by an Aboriginal or Torres Strait Islander person. The position is therefore only open to Aboriginal or Torres Strait Islander applicants.
About You
We are seeking applicants who have:
Experience in providing support to Aboriginal and Torres Strait Islander people, preferably in remote communities.
Experience maintaining positive stakeholder and community relationships.
Experience in working remote, independently and in a multi-disciplinary team environment, prioritise and meet deadlines, deal with matters of a sensitive and confidential nature.
Ability to assist and support clients to access services, reduce service complexity and confusion.
Understanding of My Aged Care and operational guidelines or the ability to quickly acquire.
Good written and interpersonal communication skills including the ability to work effectively with other service providers.
Understanding of the historical and contemporary issues that impact on Aboriginal and/or Torres Strait Islander people and their communities, particularly in Cape York.
Ability to communicate sensitively and effectively with Aboriginal and/or Torres Strait Islander people in accordance with community protocols and customs.
Demonstrated understanding of the principles of Aboriginal and/or Torres Strait Islander community control.
Must be willing and able to travel in Cape York either by light aircraft or 4WD as required.
Desirable
Qualification in Aged Care.
Experience in the community services field, preferably aged care, with strong community connections and networks.
Mandatory Requirements
Certificate IV in Community Service or Certificate IV in Community Development.
A minimum of two (2) years’ experience as a social and community worker or in a similar position.
Current Queensland Drivers Licence.
Blue Card (Working with Children Card), or willing and able to obtain prior to commencement.
AFP National Police Check, or willing to obtain prior to commencement.
