About the role
We have an exciting permanent, full-time opportunity for a Support Coordinator with a primary focus on hospital discharge processes. This position is fully remote, but applicants must be based in the Gold Coast region to regularly visit clients in Southport.

You’ll collaborate with clients to address their needs and support them in achieving their goals. You’ll work closely with them to maximise their NDIS-funded supports, connect them with suitable service providers and allied health professionals, and monitor their progress toward their plan objectives.

Responsibilities include:
• Managing complex medical needs
• Navigating hospital discharge procedures
• Ensuring smooth transitions from hospital to home
• Facilitating timely discharges
• Effectively communicating with multidisciplinary teams
• Providing comprehensive post-discharge support
• Support clients and families in accessing and coordinating services
• Implement supports outlined in NDIS plans
• Help clients develop skills to coordinate support and engage in the community
• Empower clients to understand and implement their plans
• Be responsive to client needs, especially in crises
• Conduct regular progress reviews and report to the NDIS

Skills and Experience:
• Understanding or knowledge of the NDIS (advantage)
• Prior experience in Support Coordination and/or hospital discharge coordination, or in a similar capacity within a healthcare or nursing role
• Experience with complex case management
• Strong communication skills and ability to collaborate effectively with multidisciplinary teams.
• Ability to multitask and prioritise effectively in a fast-paced healthcare environment.
• Commitment to client-centered care and ensuring smooth transitions from hospital to home or community setting

Requirements
• Valid driver’s licence
• NDIS Worker Screening Clearance (yellow card)
• QLD Blue card (working with children’s check)
• NDIS Orientation Module: “Quality, Safety & You”
• Relevant qualification in disability services, social work or similar (desirable)
• Australian working rights (with no restrictions)
