Community Care Coordinator & Lived Experience Community Care Coordinator

Position Details

  • Multiple Positions Available
  • Classification: SCHADS Level 4 with a salary range of: $85,205 - $91,686; per annum pro rata + Superannuation
  • Salary packaging arrangements providing tax benefits available for living and entertainment expenses increasing your take home income where part of your wage is paid tax free each pay, savings thousands in tax each year

About your new role

Community Care Coordinators and Lived Experience Community Care Coordinators will provide compassionate, strengths based, consumer-led and culturally appropriate care and support to people after a suicide attempt or experience of distress/suicidal crisis. Community Care Coordinators will provide short-term, one-on-one, flexible, psychosocial support and care coordination through assertive outreach. People will be supported toco-create safety plans and to develop wellbeing plans that guide their support. Support includes coordinating care with consumers, families and external stakeholders to navigate systems, services, and connection with supports available to them. Community Care Coordinators will build and maintain relationships with a number of stakeholders and community partners to facilitate holistic care for people accessing the service.

What you will bring to the role

  • Relevant tertiary qualification and/or equivalent experience and understanding of working with people experiencing distress, with mental health challenges, suicidality and/or deliberate self-harm.
  • Ability to establish and maintain positive working relationships with partner agencies and organisations to strengthen community-based prevention and intervention initiatives.
  • Strong emotional resilience including the ability to sit with discomfort, distress and people in crisis.
  • Experience liaising with external service providers, including specialist mental health services, GP's and other community-based services and organisations.
  • Personal lived/living experience of recovery as a family, friend or carer. Your recovery experience may be related to mental health, suicidal crisis, drugs and alcohol use or trauma
  • Willingness to identify or be identified by others as a professional with lived experience of recovery
  • Willingness to share and draw on lived experience purposefully in the role to instill confidence and hope in others about the journey of recovery
  • Lived Experience Community Care Coordinators will draw on their lived experience(s) of suicide and/or experiences of distress and the National Lived Experience Workforce Guidelines to provide practical, emotional, social, and service access support that explores and responds to a person's needs through a process of mutual listening, learning, doing, being, belonging and healing.

  • Nationally Coordinated Criminal History Check (police check) - disclosable outcomes considered
  • Current Working with Children Check - required before commencement of work
  • Current Australian drivers licence
  • Australian working rights
