South Coast Medical Service Aboriginal Corporation is a not-for-profit Aboriginal Community Controlled Health Organization, providing Health and Wellbeing and relevant services to the Aboriginal and Torres Strait Islander people in the Shoalhaven area.
We are currently seeking experienced Elder Care Support Coordinator to support clients to understand their aged care service entitlements, supporting identified clients to navigate access to appropriate local aged care support services.
THE ROLE
The ECS Coordinator will work closely with the Aged Care Connector to support identified clients, Nowra-Shoalhaven. They will be providing day-to-day support and supervision, ensuring quality and timely services are provided in line with organisational policies and procedures. The duties of the ECS Coordinator may vary depending on the specific needs of the customer.
The ECS Coordinator will support clients to understand their aged care service entitlements, supporting identified clients to navigate access to appropriate local aged care support services. They will provide advocacy and support to ensure clients receive a culturally safe assessment, including My Aged Care registration, contractual arrangements, and costs, and will help ensure elders and their family are well-prepared for plan reviews and outcomes.

Essential Criteria
Aboriginality*
A sound knowledge of Aboriginal/Torres Strait Islander communities and relevant organisations within Shoalhaven.
Experience working with Aboriginal and Torres Strait Islander people.
Excellent communication and interpersonal skills, with the ability to build trust with community.
Demonstrated understanding of the principles of person-centred care and trauma-informed practice.
Understanding of how to assist and support clients to access services, reduce service complexity and confusion.
Sound understanding of My Aged Care and operational guidelines.
Good record management, data collection and reporting capability.
Experience in the community services field with strong community connections and networks across aged care providers, mainstream supports and community sector.
Preferred applicants will also be required to undergo a National Police Check and obtain a new paid employee Working with Children Check (as required by the Act) prior to appointment with South Coast Medical Service Aboriginal Corporation.
Salary will be in accordance with the South Coast Medical Service Aboriginal Corporation Enterprise Agreement.
