About the Team
Our home support services help older people remain independent in their homes and communities longer. The service supports people aged 65 years and over and Aboriginal and Torres Strait Islander people aged 50 years and over who are experiencing difficulty with everyday tasks.
About the Opportunity
As the Case Manager you will work as a collaborative member of the Aged Care Team. You will operate in line with the Aged Care standards, organisational requirements, and program guidelines to develop and review care plans that enable consumers to achieve goals and enhance quality of life.
To be successful in this role you will:
- Hold relevant qualifications such as Bachelor of Nursing/ Allied Health / Social Work or Enrolled Nurse / Diploma Community Services combined with Aged Care experience
- Have prior experience in planning, co-ordination and delivery of services to older people who live at home including care assessments and the development and review of care plans
- Have demonstrated knowledge and understanding of Aged Care related reforms, legislation, compliance requirements, quality standards, the role of the Aged Care Quality and Safety Commission and the My Aged Care (MAC) portal
- Have a knowledge of the aged care and community services sector, regional service system and local providers
- Ability to work respectfully and collaboratively with Aboriginal and Torres Strait Islander peoples, their families, communities, staff, and Aboriginal controlled organisations.

Please note: It is a requirement that prior to commencing in this role, you must provide 100 points of identification to confirm Australian citizenship or eligibility to work in Australia; possess a Working with Children (Ochre Card), a current National Criminal History Check (within 3 months) and a valid NT driver’s licence.
