This blended role works across 3 programs - the NDIS (National Disability Insurance Scheme), CPS (Commonwealth Psychosocial Support) and CHSP (Commonwealth Home Support Program). We are seeking applicants who:

• Have excellent knowledge of their community or, if you're considering a move to Kalgoorlie, a natural curiosity and ability to become familiar with the people and services who make up our amazing City.
• Hold a degree level or post graduate qualification in Social Work, Community Services or other related discipline
• Are self-motivated and highly organised to maintain a busy schedule.
• Can keep excellent case notes and be efficient with the necessary administration to meet KPIs and reporting requirements.
• Enjoy being part of a larger team and building relationships with internal stakeholders, therapy partners and community support organisations.
• Are willing to apply for an NDIS Worker Check, WWCC and a National Police Certificate dated within 6 months.

Hourly rate starts at $44.29 per hour plus 12% superannuation from 1 July.
