Aligned Community Care is seeking a Team Leader – Disability Support Worker to oversee the delivery of high-quality, person-centred care to NDIS participants with complex mental health and psychosocial needs.

You’ll work closely with our Disability Service Manager to lead, mentor, and support a passionate team of Disability Support Workers — ensuring clients receive consistent, respectful, and empowering care tailored to their individual needs.

Key Responsibilities

Team Leadership & Supervision

Lead and support a team of Disability Support Workers

Provide guidance, coaching, and hands-on support

Foster a positive, inclusive, and high-performing team culture

Collaborate with the Area Manager to address challenges and support team wellbeing

Participant-Centred Care

Oversee implementation of care plans tailored to complex mental health needs

Build strong rapport with participants and support them in day-to-day life

Promote dignity, independence, and choice in every aspect of care

Address needs related to BPD, PTSD, Schizophrenia, and other mental health conditions

Compliance & Documentation

Maintain accurate case notes and incident reporting

Ensure all documentation meets NDIS and legislative standards

About You

We’re looking for a strong, empathetic leader who thrives in complex support environments:

Experience & Skills

Minimum 2 years' experience in mental health-focused disability support

Proven leadership in a Team Leader or Senior Support Worker role

Experience with participants with psychosocial and complex behavioural needs

Confident managing care for participants with BPD, PTSD, Schizophrenia, etc.

Strong communication and conflict resolution skills

Highly organised and reliable with excellent time management

Qualifications & Requirements

Certificate III or IV in Disability, or relevant equivalent qualification

NSW Driver’s Licence + reliable vehicle

Current First Aid & CPR Certificate

NDIS Worker Screening Check

Working With Children Check

Recent (within 12 months) National Police Check

100 Points of ID

Comprehensive car insurance and vehicle registration in your name
