Opportunity: Care Manager – Launceston, Tasmania

We are seeking suitable Care Manager applicants awaiting their next opportunity to join a leading not for profit Aged Care provider who is well established in Tasmania. This business critical position will be responsible for supporting, coordinating and collaborating with a large interdisciplinary team including registered staff and carers and other members of the healthcare team. You will be a motivated and supportive manager responsible for assessing and developing patient requirements and care plans, coordinating transition of care and providing appropriate support and education to patients and their families.

This position is based in Launceston. Relocation support available for successful candidates.

Key Responsibilities

Collaborate with the Facility Manager to oversee Continuous Improvement, OHS, and Training programs, ensuring care standards are met

Work with medical and allied health professionals to support positive care outcomes

Provide recommendations and coordinate referrals to specialists following clinical assessments

Monitor AN-ACC reviews and ensure accurate documentation to optimize consumer care funding

Qualifications and Requirements

Current unrestricted RN AHPRA registration

Bachelor of Nursing or equivalent qualification

2+ years of senior clinical nursing experience within Residential Aged Care

Knowledge of Aged Care Quality Standards, Code of Conduct and Charter of Resident Rights

Exercise initiative relating to organisational policies and procedures

Satisfactory national police check

Tasmanian Working with Vulnerable People registration with NDIS endorsement

Valid Australian working rights
