The Role

We are looking for an experienced Clinical Care Coordinator with strong clinical expertise to lead, support, and guide our care team in delivering high-quality clinical care and service to our residents. You will work closely with the Facility Manager to ensure resident needs are effectively assessed, documented, and met.

This role requires a clear communicator who can engage with staff, residents, and healthcare professionals. You will take a hands-on approach to problem-solving, proactively addressing challenges to ensure smooth clinical operations and the highest standard of care.

Key Accountabilities

Oversee resident clinical assessments and care planning.

Lead and manage the clinical care team, ensuring effective staffing and workflow.

Monitor clinical documentation, follow-up on incidents, and collaborate with doctors and allied health professionals.

Ensure compliance with Aged Care Quality Standards, accreditation, and policies.

Provide on-call support and strategic guidance for nursing staff.

Promote continuous improvement, clinical excellence, and quality in care.

Key Selection Criteria

Extensive clinical experience within the Aged Care sector.

Strong knowledge of Aged Care legislation, regulatory compliance and accreditation.

Demonstrated leadership skills in staff supervision and development.

Highly developed communication skills for fostering resident and staff relationships.

Working understanding of AN-ACC and the claims process.

Computer literacy in Aged Care software and Microsoft Office suite.
