About the role

Carespace Australia Support Services Pty Ltd is seeking a full-time Services Coordinator to join our team in Perth, WA 6155. As the Services Coordinator, you will play a vital role in coordinating and delivering high-quality support services to our aged and disabled clients. What you'll be doing

Coordinate and schedule the delivery of support services to clients, ensuring their needs are met in a timely and efficient manner

Liaise with clients, their families, and our care team to understand and address client needs

Maintain detailed records and documentation to track client progress and service delivery

Support the implementation of care plans and monitor client wellbeing

Collaborate with other team members to identify opportunities for service improvement

Assist with the recruitment, training, and supervision of support workers

Ensure compliance with all relevant policies, procedures, and regulatory requirements

What we're looking for

Relevant tertiary qualification in a human services or community services field

Minimum 2 years' experience in a similar services coordination or case management role

Strong organisational and time management skills, with the ability to prioritise and multitask

Excellent communication and interpersonal skills, with a compassionate and empathetic approach

Proficient in using technology, including client management systems and Microsoft Office suite

Understanding of the aged and disability support sectors, including relevant legislation and industry standards

Ability to work collaboratively as part of a team and independently as required

Current driver's licence and access to a reliable vehicle
