As a Client Coordinator working in the Aged Care Team at Rise you will manage client services within the programs of Commonwealth Home Support (CHSP), Continuity of Support (CoS) Programs. A primary focus of this role involves assessing and reviewing existing clients to ensure their needs are met and the best possible outcomes are achieved.
This position is full-time working from various Rise locations (including Helen’s Place in Middle Swan) and once up-to-speed/approved you may enjoy some work-from-home and flexibility.
A typical day might look like…
Acting as a primary contact and advocate for clients
Developing effective relationships with clients and their family and/or representatives to maximises client outcomes
Monitoring, measuring and evaluating wellness outcomes and client goals
Regularly reviewing client funding to determine if it is meeting their needs and make applications for further funding where appropriate
Developing and implementing client service plans in line with funding and legislative/contractual requirements
Maintaining tip-top documentation and creating reports as required
👉🏼 The need-to-have…
Here at Rise, we nurture potential, but we do need you to have a few tricks up your sleeve including the following:
Certificate IV in Individual Support (Ageing) and experience in residential or home care
Qualification in Advanced Diploma Community Services (desirable)
Understanding of Commonwealth Home Support (CHSP), Continuity of Support (CoS) Programs
Financial acumen and understanding/ability to meet targets
A strong commitment to providing consumer directed/ person centred care

Appointments to this position will be subject to a national police clearance. Costs for new checks will be covered and/or reimbursed by Rise.
