Based in the Strathfield area in Sydney's Inner West, our client is a long established, highly reputable community care organisation, servicing a local client base from culturally and linguistically diverse backgrounds.
An exciting opportunity has arisen to join their team, initially on a temporary basis, with a view to transitioning into permanent employment at the end of July.
The Role
The initial, 6-week temporary assignment will focus on clearing an administrative backlog and providing direct support and guidance to clients.
This will include:
- Updating client service agreements
- Engaging directly with clients over the phone to respond to enquiries and obtain information
- Checking and registering all client documents in Alayacare including care plans etc.
- Processing invoices and other documentation, and performing associated administrative tasks
It is a full-time role, Monday to Friday, commencing ASAP.
If successful in transitioning into a permanent role, your position as Care Advisor will focus on coordinating or conducting initial and ongoing client assessments within their homes, and delivering strategies, guidance and support to enhance their mobility, independence and wellbeing. This will include:
- Provision of high-quality Support at Home case management services for clients
- Undertaking or coordinating a comprehensive assessment of each client in collaboration with the client and their family or carer
- Determining health and wellbeing maintenance and promotion strategies aimed at encouraging independence and assisting the client to maintain a quality of life
- Coordinating the safe delivery of quality holistic care and support to the client, and monitoring client progress
- Engaging with allied health professionals and community supports to enhance client care delivery
- Coordinating services in line with the funding bodies' requirements, Social Justice and Access & Equity principles, WHS requirements and the organisation’s philosophy, mission, policies and procedures
- Developing associated documents and reporting, as required
What You'll Need
In order to succeed in this role, you will require the following:
- Experience in a case management role within a care focused service or setting, including experience in managing CHSP clients
- Demonstrated understanding of the needs and issues of older people and their carers
- Demonstrated experience in needs assessment and management of older people
- Excellent written and verbal communication skills
- Demonstrated ability to supervise and develop out of office staff and speak a relevant second language
- Computer literacy
- Valid National Police Check
- Valid NSW Driver's Licence and own vehicle
- Up to date COVID-19 vaccination
Although not essential, relevant qualifications would be advantageous, as would the ability to speak a second language.
The Benefits
In return, our client is offering an hourly pay rate of $50 per hour + Super for the duration of the temporary assignment (including casual loading).
The permanent opportunity will offer a commencement salary of $80-85K + Super (take home pay equivalent to $85-90K gross p.a. with salary packaging), plus a range of fantastic employee benefits including:
