Position Description

We are seeking a highly motivated Elder Care Connector (ECC) who has the primary responsibility of connecting with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of their aged care entitlements.

The overarching function of the ECS Connector is to ensure that support for clients is organised, sequenced and supported to enable effective access to appropriate aged care services both internal and external to SRAC.

The ECS Connector will work closely with the ECS Coordinator. They will also help determine the level of local demand for aged care services, identify existing aged care services, any service gaps in the area and continue advocacy work with clients as required.

Selection Criteria:

Aboriginality. This position is an identified Aboriginal position. Aboriginality is a genuine qualification authorised under Section 14(d) of the Anti-Discrimination ACT 1977

An understanding of the issues experienced by older Aboriginal people living in the community

Certificate in Community Services or Aged Care, or previous relevant experience

Familiar with local Aboriginal communities and community service providers in Nepean/Blue Mountains and Greater Western Sydney

Must have or be willing to obtain current Police and Working with Children Checks

Must hold a NSW unrestricted driver licence
