What You'll Do

Provide personalised support that helps people remain safe and independent at home

Conduct client assessments and develop tailored care plans

Deliver care services such as medication prompts, continence support, chronic disease monitoring, and general well-being checks

Supervise support staff and coordinate care delivery in line with funding and service plans

Complete reports and administrative tasks relating to care

About You

Experience in aged care, disability support, or community care

Excellent communication skills and a client-first approach

Confident working both independently and within a team

Great time management and attention to detail

Strong values around dignity, respect, and empowerment

Bonus: If you’re a Registered or Enrolled Nurse, that’s a big plus—but not essential!

Requirements

First Aid & CPR certification

National Police Check

Valid Driver’s Licence and reliable, presentable vehicle

NSW Working with Children Check

Willingness to obtain a NDIS Worker Screening Check (if not already held)
