We’re looking for a compassionate and detail-oriented professional to join our team as a Customer Intake & Reviews Officer, combining the essential work of supporting new customers with ongoing care planning and service reviews.
About the Role
This dual-purpose role facilitates new customer access to our home-delivered meals and Wellness & Social Connection services, while also reviewing existing services to ensure they continue to meet customers' evolving needs. You will conduct phone-based assessments, engage with families, branches, and external agencies, and ensure accurate documentation within customer records and My Aged Care.
Key Responsibilities
Customer Intake & Onboarding
Respond to referrals and enquiries to assess customer eligibility and service needs.
Conduct comprehensive phone-based assessments, identifying nutritional, financial, and social needs.
Develop and document clear meal plans and care arrangements.
Educate customers and families about services, rights, and responsibilities.
Collaborate with kitchens, branches, and the wider MoWSA team to ensure seamless service commencement.
Customer Reviews & Ongoing Support
Conduct scheduled reviews of existing CHSP customer services by phone.
Identify changes in customer function, risk factors, and goals, and document updated care plans.
Maintain accurate records in both MoWSA and My Aged Care systems.
Liaise with branches, family members, healthcare professionals, and other agencies.
Respond to customer feedback, welfare concerns, and adjust services as needed.
What You’ll Bring
Essential: Certificate IV in Community Services, Aged Care, Disability or equivalent experience.
Experience in customer engagement, phone-based assessments, and care planning.
Strong written and verbal communication skills, including ability to liaise across diverse groups.
High attention to detail and comfort using Microsoft Office and customer databases.
Demonstrated ability to manage competing priorities with empathy and professionalism.
Desirable
Allied Health, Nursing or Certificate IV in Training & Assessment.
Knowledge of CHSP or My Aged Care systems.
Additional Requirements
National Police Clearance (current or willing to obtain).
Current Australian driver’s licence.
