Hours: 7:30am-3:30pm (flexible)

About the Role:
As a Clinical Coordinator, you will play a key leadership role across YOURCARE’s nursing and disability programs. You will deliver hands-on nursing care (15 hours per week in the community) while overseeing clinical governance, care coordination, rostering, and team management. This is a diverse, high-impact role for a Registered Nurse who is ready to lead both on the ground and strategically.
You must be confident working across a broad geographic area, including Cleveland, Beenleigh, Brisbane North, and occasionally from our Brisbane office. Flexibility, initiative, and emotional intelligence are essential.
The successful candidate will have the availability to work on a rotating remote working roster answering calls made to our 1300 line providing guidance to field staff and clients. Generous allowance and rates paid!
Key Responsibilities:
Provide high-quality nursing care, including wound management, diabetes care, and catheter management
Deliver compassionate care in clients' homes, respecting their privacy and dignity
Travel across a wide area to reach clients, adhering to scheduled visit times
Communicate effectively with clients, their families, and the wider healthcare team
Conduct thorough assessments
Assist with training support workers
Be confident to work autonomously
Provide clinical guidance to our disability programs
Requirements:
Current registration as a Registered Nurse (RN) with AHPRA.
Competency in wound care, diabetes management, and catheter care is essential.
Must have a reliable car and a valid driver's license.
Availability to work adhoc weekends.
Ability to manage time effectively and work independently.
Strong communication skills and a commitment to client-centered care.
