Clinical Coordinator (Aged Care / NDIS) – Maternity Leave

SCHADS Award Level 5 (plus salary packaging)

Immediate start until 5 May 2026

We are looking for a Clinical Coordinator (Aged Care / NDIS) to join our team in Brisbane office. This is a maternity leave position until 5 May 2026 with a chance of extension. The Clinical Coordinator provides clinical assessments to IWA clients (aged care and NDIS) and clinical leadership to Coordinators – Case Management, Coordinators - Rostering, and support staff. The role ensures the continued success of home care support programs and overall team coordination at IWA Australia.

As a Clinical Coordinator at IWA Australia, your main tasks and responsibilities will be:

Assist in the submission of referrals to Home care and NDIS access services or higher levels of home care packages.

In conjunction with clients, significant others and health professionals, conduct clinical assessments of all IWA client admissions and reviews, meeting program-specific requirements

Liaise with the Coordination Team to make clinical recommendations of supports to aged care / NDIS clients.

In consultation with clients, their representatives and health professionals, develop a comprehensive support plan that meets the clients’ clinical and holistic needs

Regularly review and update the care plan to ensure effective service delivery and compliance with quality standards

Ensure that all clinical documentation is completed accurately and in a timely manner in accordance with IWA’s policies and procedures.

As a member of the clinical team, make decisions regarding questionable items posed within the Inclusions/Exclusions Framework of the Aged Care- Home Care Package Program

Assist in the development and review of clinical policies and procedures

Record and follow up on incidents / complaints as required.

Assist in maintaining IWA’s clinical governance framework and compliance

Provide ongoing clinical leadership, advice and guidance to Coordinators – Case Management, Coordinators and support staff.

Assist in developing a training calendar for support staff.

Prepare & submit monthly clinical team report to the Quality Advisory Committee

Key Selection Criteria:

A qualification/experience in a relevant industry area

Well-developed oral and written communication skills including an ability to liaise effectively with a people from diverse backgrounds

Ability to work both independently and in a collaborative team environment

Ability to assess priorities and manage competing deadlines both independently and as a member of a team

Demonstrated high level literacy, numeracy, accuracy and attention to detail skills.

Excellent interpersonal and communication skills, including the ability to consult, negotiate and liaise effectively with a diverse range of people
