Key Responsibilities:

Determine individual client needs and organize, create, and implement tailored support, training, and educational initiatives.

Assist clients and their families or caregivers with activities of daily living such as self-care, personal hygiene, cooking, budgeting, skill development, and community participation to achieve goals outlined in their care plans.

Facilitate access to external services and resources, including healthcare, employment services, and social networks.

Track and report on clients’ progress, ensuring that care and support plans are reviewed and updated regularly.

Evaluate and communicate evolving client needs to management to ensure effective planning and resource allocation.

Document service delivery activities and outcomes in line with organizational and regulatory requirements.

Assess community needs and available resources, particularly in the areas of housing, employment, welfare, and healthcare.

Assist in planning and implementing community-based initiatives to enhance access to services and strengthen social inclusion.

Build partnerships with community organizations, local councils, and service agencies to deliver collaborative projects.

Provide care, education, and support to children and individuals with disabilities in various settings including group homes, adult services, and government-operated facilities.

Support families and caregivers by offering practical assistance, emotional support, and referrals to relevant services.

Advocate for the rights and needs of individuals living with disabilities, promoting independence and community participation.

Collaborate with family members, case managers, clinicians, and service providers to develop and implement individualized care plans.

Liaise with internal and external stakeholders to coordinate services and ensure client needs are met holistically.

Contribute to team meetings, service reviews, and planning processes to promote continuous improvement.

Key Skills and Competencies:

Strong communication, advocacy, and interpersonal skills

Experience in case management, client assessment, and community engagement

Knowledge of the social services, disability, and mental health sectors

Ability to work independently and within multidisciplinary teams

Commitment to diversity, inclusion, and empowering vulnerable populations
