About the position
Working collaboratively with other Grampians Community Health staff, the Community Health Nurse delivers accessible nursing services and health education tailored to meet the priority needs of our community. This role focuses on providing comprehensive health assessments, planned care, and preventative health education for individuals who may face barriers to accessing mainstream health services.
The position offers scope for the nurse to respond to a wide range of general health issues, supporting clients to improve their overall wellbeing. In addition to this broad focus, a component of the role involves delivering reproductive health care and education including, but is not limited to, reproductive health promotion, clinical support, and referral pathways for individuals.
This is a dynamic and responsive role, ideal for a nurse passionate about preventative care, health equity, and community empowerment.
Skills and experience
• Registered Nurse (AHPRA registration)
• Post-graduate qualifications in Community Health Nursing or related field.
