East Wimmera Health Service (EWHS) is a fully accredited small rural health service across five campuses encompassing the five communities of Birchip, Charlton, Donald, St Arnaud and Wycheproof, in North -western Victoria.
East Wimmera Health Service is now seeking permanent part time Central Intake Nurse to join our expanding Community Health team.
As a member of a multi-disciplinary service, the Central Intake Nurse takes responsibility for referrals entering our Community Health services and ensures comprehensive health screening via telephone to determine; services and assistance required, priority, eligibility and costs of service.
The clinician’s key role is to:
· Conduct comprehensive initial needs assessment (INI) of the restorative, physical, medical, psychological, cultural and social dimensions of care and provides consumers and carers with a choice of appropriate services.
· Provide information and refer clients to services both internally and externally that are appropriate and available to meet their needs and preferences.
· Provide education and support of those entering service including educating of pathways to receiving subsidised service such as post-acute care (PAC), hospital in the home (HITH), National Disability Insurance Scheme (NDIS) and Commonwealth Home Support Program (CHSP)
