ABOUT THE ROLE

The role of a Support Coordinator & Psychosocial Recovery Coach is to provide individualised support to participants (who have a disability and/or mental illness) of the National Disability Insurance Scheme (NDIS). The Support Coordinator & Psychosocial Recovery Coach will work with the participant to implement the supports within the person's plan for the purpose of building capacity, managing the impact of their disability, and improving their health, well-being, and participation within the community.

Experience as a Support Coordinator/ Psychosocial Recovery Coach is essential.

This role is a 12-month contract and will then be reviewed to a permanent role. The contractor is scheduled to begin their employment in July 2025.

The role is combine working from our office (Hawthorn) and working from home with your clients based in the North/West Melbourne region.

WHAT IS REQUIRED FROM YOU:

· The ability to take a coaching approach with your participants to build their capacity in managing their own NDIS plan.

· The ability to deliver high-quality services.

· The ability to complete mandatory reporting to the NDIS and child protection.

WHAT YOU WILL NEED:

· The Senior position requires no less than 2 years experience as a Support Coordinator/Psychosocial Recovery Coach in the field.

. The Junior position requires 6-12 experience working as a Support Coordinator or in a related field with the NDIS, Child Protection, or other similar department.

· A minimum of certificate IV/Diploma in Disability, Mental Health, Community Services.

· Right to work in Australia

· Satisfactory completion of safety screening including a National Police check, International Police check (if required), a NDIS Working screen check

· A current Victorian Working with Children Check (Employee),

· Current Victorian Drivers License
