Healing voices offers Support coordination and Recovery Coaching services for participants Australia wide. We are seeking an experienced support coordinator to join our team, for an immediate start.

The nature of many of our referrals is often complex. We are looking for a Support Coordinator who has experience supporting participants that have mental health, psychosocial or disability support needs in a case management style role.

This is a casual role with potential to grow to full time as more clients are onboarded. Your hours will be determined by your caseload.

You will work proactively with participants by coordinating and engaging supports to ensure they are getting the most out of their NDIS plans.

You will have excellent communication and report writing skills to support plan reviews and get the best outcome for participants.

You will Participate in community engagement & networking and promotion that leads to measurable outcomes and increased numbers of client engagement.

To be successful in this role you will have:

Minimum of 2 years of current experience in a Support Coordination Level 3 role.

Understanding of the role of Support Coordination within the NDIS framework and the interplay with broader systems (health, education, child protection, mainstream supports)

Preferable justice liaison experience

Extensive knowledge of disability, mainstream and community services.

Be able to articulate a clear and ethical practice framework

Qualification in Social Work, Social Science, Health Science, Case Management, Mental Health/Psychology or other Allied Health qualification.

Demonstrated knowledge of psychosocial diagnoses, both chronic and acute, or the ability to source accurate and critical patient information.
