About the role
Healthcore is seeking an enthusiastic, well organised Client Intake Officer to join our Intake and Scheduling Team. In this pivotal role, you will be the first point of contact for clients and referrers, ensuring an exceptional customer experience and seamless intake process.
Key responsibilities
Receive, process and triage new client referrals
Accurately collect and record key clinical and demographic information (diagnoses, risks, barriers, goals)
Coordinate initial appointments and support caseload optimisation for our clinicians
Prepare and manage intake documentation, including NDIS Service Agreements
Maintain accurate and timely records within our CRM and scheduling systems
Liaise with Clinical Managers to ensure referrals are allocated appropriately
Match clients to therapists based on clinical needs, goals, and geographic location
Assist in managing urgent or complex referrals in collaboration with management
Build and maintain strong professional relationships with referrers and external stakeholders
Contribute to continuous improvement initiatives across intake and scheduling processes
About You
You may come from a background in NDIS Support Coordination, allied health administration, medical reception, community services or a related field. You are highly organised, confident communicating with a wide range of stakeholders and take pride in creating positive first impressions. You enjoy working in a collaborative team environment, can juggle competing priorities with professionalism, and value meaningful work.
Desirable experience and attributes
Previous experience in a similar intake, administrative, or support coordination role
Strong understanding of the NDIS, My Aged Care, or community health systems
Excellent verbal and written communication skills
Highly organised, with strong time management and multitasking ability
Sound knowledge of allied health services (e.g. OT, physiotherapy, dietetics, speech)
Confidence using CRM systems and Microsoft Office Suite
Strong attention to detail and a proactive approach to process improvement
