About Us?
The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The Quality and Innovation Unit provides leadership in the development, implementation, and evaluation of
plans, programs, models, and policies for improving Aboriginal and Torres Strait Islander Health and Well- being. The Unit will develop and manage to maturity enhanced service delivery models in new and expanded functional IUIH services and programs.
About the Role?
The purpose of the position is to be a comprehensive linkage and coordinate services to address gaps in Palliative Care and End of Life Care for Aboriginal and Torres Strait Islander people at the interfaces between home/community, primary care, and secondary and tertiary care service in Southeast Queensland.
The position will work within the Palliative Care Services Team and assist in the implementation of a new service designed to meet the unique needs of Aboriginal and Torres Strait Islander individuals and their families. As part of multi-disciplinary team, the position ensures Palliative Care and End of Life social health services, training, supports, and information are delivered in accordance with the IUIH Model of Care.
The Palliative Care Coordinator will support the Project Team and Member Services to connect services and partnerships. The role will influence the development of the service to effectively deliver resources, avoid duplication, and build sustainability and scale.
This role also provides social and emotional support and health education to clients and family members throughout the client’s Palliative and/or End of Life Care journey. Also, to work effectively with other social and health practitioners to identify and access appropriate services and supports to meet the client and family’s needs.
Day to Day Duties may include:
- Provide advocacy, support, and linkage for Aboriginal and Torres Strait Islander clients with a life limiting illness.
- Walk alongside our clients & families to enhance the Palliative Care and End of Life journey.
- Provide day to day health service activities within the context of Palliative Care and End of Life Care in a professional, confidential, and culturally safe manner.
- Collaboration with the project team and Member Services to identify ways to connect to existing programs and services across Southeast Queensland to develop a strength focused plan that sets out priorities and actions, including defining support/interventions from other social health providers and network clinics and services.
- Supporting the mapping and scoping of the existing workforce skills, capacity, and training needs and options.
- Initial triage/assessments where appropriate of referrals for Palliative Care equipment – via telephone or face-to-face as required.

- Current C Class Drivers Licence (Qld)
Desirable, but not mandatory:
- Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
