About Us
Apunipima Cape York Health Council is a community controlled Aboriginal and Torres Strait Islander Health Service that provides culturally appropriate, family centred, primary health care services to the communities and people of Cape York.
About the Opportunity
The ITC Care Coordinator is responsible for facilitating access to the ITC Program for clients with chronic health conditions who live in Cape York communities. The position is fulltime, fixed term with an end date of 30 June 2026
This position is identified where, under section 25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander community.
About You
To be successful in this role you will have:
Experience in chronic conditions management and care coordination.
Experience advocating for access to services on behalf of clients.
Experience working collaboratively with other health care providers to deliver or facilitate access to quality health care for clients.
A working knowledge of clinical best practice in relation to chronic conditions management preferably in remote settings.
Demonstrated understanding of integrated and collaborative client care within a multidisciplinary primary health care team.
Understanding of the historical and contemporary issues that impact on Aboriginal and/or Torres Strait Islander people and their communities, particularly in Cape York.
Ability to communicate sensitively and effectively with Aboriginal and/or Torres Strait Islander people in accordance with community protocols and customs.
Demonstrated understanding of the principles of Aboriginal and/or Torres Strait Islander community control.
Must be willing and have the ability to travel in Cape York either by light aircraft or 4WD as required.
Mandatory requirements
Certificate IV or Diploma in Aboriginal and/or Torres Strait Islander Primary Health Care.
Current Queensland Driver's Licence.
Current Blue Card positive notice or exemption, or willing and able to obtain prior to commencement.
Current AFP National Police Clearance or willing to obtain prior to commencement.
