About Us?

The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.

The Quality and Innovation Unit provides leadership in the development, implementation and evaluation of plans, programs, models and policies for improving Aboriginal and Torres Strait Islander Health and Well-being. The Unit will develop and manage to maturity; enhanced service delivery models in new and expanded functional IUIH services and programs.

About the Role?

We are looking for Mob to work alongside members of our community who have been marginalized by the system including Mob experiencing homelessness, leaving prison and who are experiencing mental health issues including suicidal crisis and after a suicide attempt.

As a Care Coordinator in our team, you will be supporting Mob to connect with community, services and health and social supports to strengthen their autonomy and health and wellbeing. Our focus is around strengthening supports for Mob to foster social and emotional well-being to support Mob so are strong, connected and deadly.

The role will manage a caseload of clients with varying needs and identified issues, to empower and foster strength, resilience and well-being through participation in intensive case management, planning and coordination of transitional needs.

  • Work collaboratively as a member of a multidisciplinary team to provide integration and coordination between different programs
  • Developing and maintaining plans in conjunction with clients and relevant service providers (i.e. ‘Stay Strong’ Plan, GEM, Transitional Care Plan, Safety Plan)
  • Work with clients in a trauma informed, recovery and/or strengths-based approach to formulate, implement and review well-being plans including a comprehensive assessment of need and safety planning
  • Assisting in the coordination and delivery of services for clients identified with drug and alcohol, mental health, chronic disease and social health issues
  • Conduct home, community and/ or custody visits to support ongoing client recovery

About You?

· Ability to work sensitively and effectively with Aboriginal and Torres Strait Islander communities, their leaders and health professionals, while respecting traditional culture, values and ways of doing business.

· Current C Class Drivers Licence (QLD)

· Current Criminal History Check

· Valid Blue Card

· Valid Disability Worker Screening Check (Yellow Card)

· Certificate IV or Higher in Aboriginal and/or Torres Strait Primary Health or willing to complete

· Current CPR and First Aid Certificate

· Prison access clearance (obtain within 6 months of commencement)

· Custodial Awareness Training
