About the role
As a Care Coordinator in our social health team, you will provide coordination of supports to Aboriginal and Torres Strait Islander clients who experience complex mental health and social and emotional wellbeing needs. Within the role, you will work closely within the multidisciplinary team to identify and access appropriate services and supports and provide short term brief interventions as required. This position is embedded in the MATSICHS Caboolture Clinic and works collegially with Counsellors, Psychologists, Wellbeing Support Workers and other primary health care team members ensuring optimal client outcomes.
Your day-to-day duties may include:
- Maintain Social Health Pathway Intake Register and actively manage allocations and waiting lists for Social Health supports
- Undertake the intake, assessment and triage process and be a central point of contact and access for clients, particularly those with multiple, complex presenting issues.
- Provide support to acute/crisis presentations, either in a clinic or outreach capacity
- Undertake comprehensive assessment of clients’ psychosocial needs in consultation with the client and their closest supporters to develop a strengths based recovery plan that sets out priorities and actions
- Provide direct support and advocacy to facilitate care coordination goals
- Assist in the coordination and delivery of services for clients identified with alcohol and/or other drugs and/or mental health issues, in accordance with a care coordination plan and/or team care arrangement
- Provide education and culturally appropriate services to clients with identified alcohol and/or other drugs and/or mental health issues and suicide prevention strategies, which include risk assessments, triage, support, brief interventions, and care coordination
- Actively lead and participate in regular multidisciplinary team reviews of care with a focus on supporting effective client transitions through the service
About you
- Current C Class Drivers Licence (QLD)
- Current Criminal History Check
- Current Blue Card
- Tertiary level qualifications in a mental health/AOD or related field
- Minimum three years’ relevant experience including direct client service delivery in mental health/AOD and demonstrated ability to provide evidence based assessment and intervention.
