WeSupportU is a registered National Disability Insurance Scheme (NDIS) Service Provider established in 2018, servicing Bayside and Peninsula Melbourne. From a few dedicated local health workers believing they could do the NDIS differently, WeSupportU now employs more than 50 multi-disciplinary team members. ABOUT THE ROLE
WeSupportU’s Specialist Support Coordinator and Psychosocial Recovery Coaches support a dual case load of Specialist Support Coordination and Psychosocial Recovery Coaching participants.
The Specialist Support Coordinator component provides comprehensive support coordination to participants with complex needs, behaviours of concern, medical needs, or other high-risk circumstances; and works with participants to resolve points of crisis, assist in risk management and provide informed choice and decision making as the interface with mainstream and community services ensuring high quality service integration. The Psychosocial Recovery Coaching component focuses on providing specialised capacity building supports to people with a psychosocial disability, through strong and respectful relationships and skilled coaching.
Specialist Support Coordinator and Psychosocial Recovery Coach utilise their qualified expertise, knowledge and experience to guide staff and lead the delivery of innovative and person-centred supports.
Full-time, Part-time, Casual considered.
Attendance at the office (Cheltenham and/or Frankston locations) is required however there are options to work a hybrid model following induction.
TO BE SUCCESSFUL IN THE ROLE YOU WILL HAVE THE FOLLOWING:
Tertiary qualifications in Psychology, Social Work, Occupational Therapy, Nursing/Health Sciences or equivalent discipline; combined with or including relevant qualifications of a Certificate IV in Mental Health or Mental Health Peer Work or equivalent training
Current membership or practicing eligibility to join relevant regulatory body to your degree (eg. AASW, AHPRA)
A minimum two years of experience in related field of practice
Demonstrated experience in providing case management, case coordination and risk management to individuals with complex support needs
Genuine interest and passion for working with people with a disability
Ability to work independently and as part of a multidisciplinary team
A working knowledge of the NDIS, and experience in working with participants to
achieve their goals in line with their NDIS plan
A commitment to evidence-based assessment and intervention approaches
Willingness to undertake ongoing training relevant to the position
Effective collaboration and team membership to support implementation of strategies and plans
The ability to manage challenging situations
Computer literacy in Word, Excel, client databases and the Internet
Ability and willingness to travel within the Greater Metropolitan area
Other relevant Information
The incumbent must hold work rights to hold a permanent full-time position in Australia
A current Victorian drivers’ licence, own vehicle and willingness to travel throughout greater Melbourne is required
This role requires satisfactory pre-employment compliance evidence including Working with Children Check, Police Check, NDIS Worker Screening and evidence of Covid 19 vaccination
The incumbent is required to participate in the annual Goals and Performance Review program
Pay and award
Specialist Support Coordinators are employed under the Social Community Home Care and Disability Support (SCHADS) Award, Level 5. Pay point is dependent on qualifications and industry experience. At the end of each 12 months of continuous employment, an employee will be eligible for progression from one pay point to the next within a level, if the employee has demonstrated competency and satisfactory performance over 12 months.
