Are you a passionate and experienced Registered Nurse looking for an opportunity to make a real impact on the lives of older Queenslanders? We are seeking a dedicated Care Manager Community Nurse to coordinate and enhance our aged care services in the Southeast Queensland region.

Your Role:
As Care Manager Community Nurse, you will be a key member of our team, responsible for coordinating and delivering high-quality, evidence-based care to clients in their own homes. Our programs are designed to support early hospital discharges and prevent unnecessary admissions by providing comprehensive in-home care.
You will apply your clinical expertise to manage a diverse and complex caseload, ensuring that each client receives personalised care tailored to their needs. You will have your own client caseload to manage, including initial and ongoing case management and relevant reporting.
Key responsibilities include:
Clinical Leadership: Deliver high-level clinical care, including wound care, fall prevention, and risk assessments.
Case Management: Oversee initial and ongoing client case management, ensuring care plans are adjusted to meet clients’ evolving needs.
Team Collaboration: Work closely with medical professionals and allied health services, including Nurse Navigator programs, as well as other QAADS team members.
Client-Centred Care: Monitor, implement, and modify care plans in collaboration with clients and families, empowering them to achieve personal and clinical goals.
Policy Development: Support the development and improvement of policies and procedures for care workers to ensure best practices and compliance with industry standards.
What We’re Looking For:
Essential:
Current AHPRA registration as a Registered Nurse.
Experience in multidisciplinary teams within a healthcare setting.
Proven clinical expertise in hospital discharge planning, case management, and client risk assessments.
Strong problem-solving, clinical reasoning, and communication skills.
A client-focused approach with an ability to manage priorities in a fast-paced environment.
Desirable:
Two years of experience with aged care and home services.
Knowledge of relevant industry regulations and quality standards.
